NEW PATIENT REGISTRATION
To register as a new patient, contact the surgery on 020 8538 2858, or visit the practice and ask one of our receptionists. New patients are asked to complete to complete all forms from the links below, this is because it takes time for us to retrieve your medical records.

A health check must be booked a month in advance of your application before you are completely registered with our practice, during the health check you will be asked to provide a urine sample.

Everyone 5 and over should have the new patient health check.

New patients who fulfil government criteria of eligibility to NHS care are able to register with this practice, after an application process, with this practice.

  • All new patients over the age of 5 years will be offered a health check appointment with the practice nurse.
  • Patients over the age of 75 are seen annually in the surgery or visited at home.
  • All patients over the age of 75 now have a named GP at L.L. Medical Care Ltd (Agarwal & Agrawal Practice). Patients can enquire about this at our reception.

You will also be asked to provide;

  • Your medical card (from GMS1 to be completed if this is not available).
  • Identification - Photographic driving license/Passport, ID card, birth certifcate, and proof of address - utility bill (Council tax, household bills (dated within 3 months)
  • If you are registering children under 10 years of age, you must provide copies of all immunisations
  • If you have had any previous medication, or have had a change of medication, remember to bring copies of your green form, which is attached to every prescription, on request

ONLINE PATIENT REGISTRATION
Our practice is now offering online registration, this is open to all residents in Waltham Forest. Please check that your postcode is within our catchment area.

To register, vist https://www.eastlondonregistergp.nhs.uk, enter your postcode and choose from the selection. You will be asked to fill in a form and submit your personal information so we can register you.

Your registration will be registered within 2 working days.

If you want to come in person to register, we will be happy to help you, please contact the surgery for more information. You do not have to show any ID or proof of address to register.

IMPORTANT NOTICE ABOUT BOOKING APPOINTMENTS (FACE-TO-FACE, ONLINE AND PHONE BOOKINGS) - CORONAVIRUS
Please do not book a GP appointment or attend L.L. Medical Care Ltd if;

  • You have symptoms of cough, fever or difficulty in breathing after being in contact with someone with a confirmed case of coronavirus
  • You feel that they may have Coronavirus or may have come into contact with someone who is or has shown the symptoms of the Coronavirus

NHS 111 has created Coronavirus pages to inform patient's on what to do based on the above.

Due to the increase in Coronavirus, all patient appointments will now be telephone based triaged where an appointment can be made via the telephone on online. Any face-to-face appointments are at the GP's discretion. Alternatively use the eConsult service which we will respond back to you. Online appointments are currently cancelled for the time being.

Any patients who are exhibiting symptoms of the Coronavirus will not be seen and advised to visit the practice.

Prior to the appointment all patients are required to have their temperatures checked and to ask the receptionist for a token which they can use on our blood pressure machine, the machine will check your Blood pressure, height and weight. You will get a printout of your results which can then be passed onto the receptionist where it can be added to your health records.

On-entry to the Health centre you will be asked by the health centre security guard to make sure that you are wearing face masks or face coverings, he will ask if you are exempt, and will ask you to santatise your hands. The service is provided by Prototech Security.

Face masks and coverings are to be worn at all times by all GP's, nurses, visitors and patients.

FACE MASKS AND COVERINGS EXCEPTIONS - IF YOU ARE MEDICALLY EXEMPT FROM WEARING A MASK OR FACE COVERING

  1. Exception from face covering badges (for a mobile phone)
  2. Exception from face covering badges (to print)
  3. Exception from face covering card (to print)

Some people may feel more comfortable showing something that says they do not have to wear a face covering. This could be in the form of an exemption card, badge or even a home-made sign.

COVID-19 VACCINE

  • If you are aged 18 and over
  • You are at high risk from coronavirus (clinically extremely vulnerable)
  • If you are a frontline health and social care worker
  • You have a condition that puts you at higher risk (clinically vulnerable)
  • You get a carer's allowance , get support following an assessment by your local authority or your GP record shows that you are a carer

If you think you are an eligible unpaid carer who has not been contacted for your coronavirus please contact one of our receptionists.

We urge everyone eligible who has not yet had their first vaccine to book on the national booking system or by calling 119.

  • You can book an appointment at a community pop up clinic or at Leytonstone Library, or make general vaccine enquiries by calling 0208 496 3000
  • You can book an appointment at Chingford Leisure Centre, Westfield or at a pharmacy site on the national booking system or by calling 119
  • If you have had your first vaccine, please make sure you have your second vaccine to ensure maximum protection
  • If you have specific concerns about taking the vaccine and would like to talk to a health professional please contact your GP

GETTING THE HELP YOU NEED
Most consultations are over the phone and via video. This is for everyone's safety. We will assess your situation and invite you into the practice if needed.

We can arrange same-day appointments if you need urgent care but please don't visit without an appointment. Appointments will be held in the best way to keep you safe from Covid.

You can also contact 111, day or night, for other urgent health advice - including if you have concerns about your Covid-19 symptoms.

Please remember that 999 and A&E are for emergencies.

BOOKING APPOINTMENTS FOR 8 WEEK (2 MONTHS) PARENT AND BABY IMMS
During the Coronavirus pandemic, all young babies will continue to receive their 2 month (8 week) checks and immunisations from the practice. Patient's should contact the practice to make an appointment for themselves and their baby or babies, with one of our GP's and nurses. Post-natal appointments can also be made with the nurse.

We are sorry that we can not sign passports anymore, this is per the passport office.

The signing of passports can not be signed by a doctor as they have to state that they are a friend of the applicant.

Passports need to be signed by a professional person who is a friend of the applicant.

This is not the doctor’s decision, but the decision made by the passport office.
